4 Antihypertensives meds and still have a elevated BP? 200/120?

I have a sister who is 32 years old, is 5'7'' and weigh 233 in which she recently lost give or take a few 70 pounds within a year. She has hypertension and she is taking Catapres 0.2 mg, Metoprolol, Lisinopril and HCTZ everyday and some she take 3 times a day. Today she went for a routine check up since almost a couple months and her BP was 200/120. The doctor sent her to ER and they monitored her BP and it was going up to 210/134. Initially i thought ok white coat hypertension. But later she told the doctor that her systolic BP doesnt go below 190 she checks is regularly. They gave her Catapres 2mg and also something IV surrounded by which i dont remember. She is also a smoker in which she started smoking this year. She has no symptoms no headache, dizziness, blurry vision. I am convinced her BP wont go down due to smoking. But besides smoking what could also be the pretext for FOUR antihypertensives meds not help BP go down? shes also out of work so i guess that adds stress but her BP is extremely high

stop smoking, HBP doesn't have symptoms, it's the silent slayer.
Still needs to lose wt, smoking/nicotine is an appetite suppressant, initially. later, years then, it catches up with you, gain wt anyway. check thryoid.
